ABI - Acquired Brain Injury
A smaller department adjacent to our main site in Holland centre, ABI provides residence and direct support to individuals living with acquired brain injuries. Individual's goals are geared toward achieving the greatest level of independence and self-direction possible. As a group, they make decisions and work together to maintain their living spaces, prepare meals, and plan activities.

Supported Group Living
Our congregate living program provides service and support for individuals with complex developmental and medical needs. Our fully accessible facilities and 24/7 nursing staff allow us the ability to support individuals whose medical or physical needs would make independent living more difficult. It is our mission and our passion to support individuals to reach their goals and achieve true quality of life.
How to Access Service through HCCSS or DSO
Outreach
The Outreach program is designed to help self-directed individuals who require some assistance to be able to remain in their own homes. Outreach workers travel to supported individual's residence to provide support up to a maximum of 21 hours per week.
An offshoot of the Outreach program is our bathing program, which is available in Markdale and Hanover.

Apartments
Our apartment programs are designed for individuals able to live independently and direct their own care, but who may require some assistance. Staff are available on a 24/7 basis for such assistance as is required with household tasks and/or personal care, but supported individuals live independently and manage their own finances, medications, healthcare etc.

Respite
Individuals supported elsewhere (often in their family homes) may apply to stay in the respite room for periods up to one month. This service is intended to provide a resource for caregivers by providing a safe place for supported individuals to have a temporary stay.

Owen Sound Community Supports
Owen Sound Community Supports provides 24/7 support to individuals residing in community homes, providing assistance with the tasks of daily living as required by the person in question. Supported individuals have individual support plans designed to help them reach their goals, participate in their community, and continue to gain skills and enrich their lives.
