We are currently looking for PSWs or DSWs for our main site in Holland Centre, home to 22 supported individuals. While we are considered a "large group home", our small size allows for more individualized support and provides a more calm and flexible work environment compared to large care facilities or hospitals.

Community Support Worker - 525 Program
Our dedicated team at Participation Lodge Grey-Bruce is committed to helping individuals with complex needs achieve a true “quality of life” which goes well beyond the basic needs of personal care. Each supported person has their own physical and emotional needs and we strive to meet those needs on a daily basis. We have an amazing group of staff providing support for our clients and their choices, throughout our various programs and locations across the Grey-Bruce region.
Salary: $27.16-$28.15 per hour
This program is 2:1 and 1:1 support team for a 24/7 community-based client located in Owen Sound. We are looking for 1 full-time, 2 part time and 2 casual support workers to join our team!
The successful applicant needs to be willing and able to work day, evening and night shifts, consisting primarily of 8-hour shifts.
The Ideal Candidate will possess:

DSW training is preferred, but we are happy to consider all applicants experienced with complex needs and behaviour management
-
Preference will be given to applicants with Autism Support experience
-
All employees must be able to diligently follow a Behavioural Support Plan in order to ensure client success, and be able to work as a partner with the client’s family advocates
-
Clear criminal background check and valid First Aid/CPR certification
-
Valid G drivers license with clean 3-year abstract
-
Proof of vaccination against Covid-19 is required, consideration for those with medical exception

The ability to work independently and excellent communication skills to collaborate with the rest of the support team

You will:

Support the person in accessing and participating in their community.

Support the person in daily activities such as shopping, cleaning preparing for appointments and outings,

Support the person with personal care and hygiene needs.

Assist the person in developing new skills including daily living skills, communication skills and social interaction skills.

Facilitate self-advocacy and empowerment.

Support the individual to access and maintain work, or other experiences.
You will:

Learn to develop person-centred working relationships with Supported Individual and their family in their daily lives through:

Taking personal responsibility for learning about the Supported Individual (SI)

Listening to the SI and working with the Substitute Decision Maker (SDM) in planning and day to day decisions

Reflecting on own practice to continuously develop knowledge and skills.

Assisting the SI to develop and maintain a high standard of personal hygiene and appearance.

Supporting SI to make appropriate choices regarding their nutritional needs.

Administering medication as directed, in accordance with SI needs and taking full account of the Medication Policy and Procedures

Actively encouraging and supporting leisure and meaningful activities both inside and outside of the home, in accordance with SI interests, choices and needs.

Adhering to all safeguarding requirements and any procedures aimed at the protection of SI and staff.

What the position entails:

Act as charge nurse, leading the team in providing exceptional person-centered supports

Qualifications:

Outreach
Participation Lodge Grey Bruce is seeking a reliable and self-motivating individual to join our Outreach Program to provide assistance to an individuals in Wiarton or Hanover. The purpose of the role is to enable individuals to live independently while having the extra supports they need for day-to-day tasks.
Salary: $22.77 per hour
What the position offers:

15 hours per week, depending on the needs of the client

Flexibility for scheduling, within reason
-
Mostly daytime hours, no overnight shifts required
Day-to-day responsibilities:

Communicate with the supported individual to determine task priorities

Assist with meal preparation

Perform light housekeeping and cleaning duties

Non-medical personal care assistance

Participating/supporting in hobby time, when requested

Other duties as required/requested
The ideal applicant possess:

PSW/DSW certification, preferred but consideration may be given to relevant working experience or combination of personal experience and post-secondary education

Valid First Aid & CPR Training

Current NVCI training, or willing to attend in-house training

Dependable transportation within the designated region, personal vehicle preferred

Excellent communication skills with an emphasis on diplomacy
